Dear Colleague,

Below is an update on the 2 July 2013 Russian Federal Proton M/Block DM-03 GLONASS mission failure.

Russian Investigation and FROB Status
The Russian State Commission has completed its review of the Roscosmos Commission findings. A translated summary of the findings is expected to be provided to ILS on Saturday, August 3.

The summary includes information pertaining to the improper installation of the three yaw angular rate sensors located on the Proton launch vehicle and details that the premature Loss of Contact (LOC) of the electrical/pneumatic launch pad interface was not a contributing factor to the failure. An official ILS/KhSC public release of the Russian State Commission findings based on the summary information provided will follow, along with a schedule for the upcoming Failure Review Oversight Board (FRO:cool:.

Based on the progress to date, it is expected that the Failure Review Oversight Board (FRO:cool: will begin on August 9 and will tentatively conclude by August 16.

Astra 2E Campaign Status, Return to Flight and Proton Manifest
The Astra 2E team will resume campaign operations after the Russian State Commission and ILS FROB complete the required work on root cause and required corrective actions and all parties agree to re-start the campaign.

The schedule for the return to flight and subsequent missions will be determined once the Russian State Commission and ILS FROB complete their efforts and the required corrective actions have been implemented. A public release will be issued at that time.

MOSCOW, August 6 (RIA Novosti) – Launches of light-class Rokot carrier rockets will resume in the first half of September following a nine-month suspension due to attempts to fix a glitch in the rocket’s booster, Russian space agency, Roscosmos, said.
The glitch was detected during a Rokot launch in January when the rocket’s Briz-KM booster failed to deliver three military satellites into their designated orbits. As a result, one of the satellites was lost.
Roscosmos chief Vladimir Popovkin told reporters Monday that the glitch had been fixed and the next launch of a Rokot vehicle carrying Gonets low-orbit telecoms satellites will be conducted in early September.
Russia’s space program suffered a series of setbacks in the past few years. In a most recent incident, a Proton-M rocket carrying three satellites for the Glonass positioning system, Russia’s answer to GPS, crashed in a ball of flames, seconds after blasting off from Russia’s Baikonur space center in Kazakhstan on July 2.
Deputy Prime Minister Dmitry Rogozin heavily criticized Russia’s space industry on Monday, lambasting technical incompetence and calling for structural reform across the whole sector during a meeting of a commission appointed to investigate the Proton launch failure.
In July, Russia's Audit Chamber said the Federal Space Program was ineffective, and blighted by poor management and use of funds for space projects.

ILS FAILURE REVIEW OVERSIGHT BOARD TO CONVENE

August 8, 2013- A summary of the findings on the root cause and corrective action plan following the failure of the July 2 Proton M/Block DM mission with three GLONASS navigational satellites for the Russian Federal Government, has been released to International Launch Services (ILS) upon being cleared by Russian security.


The summary indicates that the most probable root cause of the failure was due to the improper installation of the three yaw angular rate sensors located on the Proton launch vehicle, which caused the vehicle to deviate from its flight path shortly after lift-off.


The ILS Failure Review Oversight Board (FRO:cool: will begin on August 9 and will provide an independent review of the investigation, probable root cause and corrective actions required prior to return to commercial flight, in accordance with U.S. and Russian government export control regulations. The FROB will tentatively conclude by August 16.


The FROB consists of ILS customers, industry subject experts, and insurance industry representatives. After the conclusion of the FROB, the FROB report will be briefed to ILS customers and the launch insurance industry.

Additional information will be provided as it becomes available.

So, does "probable cause of failure" mean they don't really know? And no mention of why lift off was before T-equals-zero and any affect that may have had.

Doesn't do much for client confidence!!!
I don't think I would want my multi-billion dollar sat riding up on something where the fatal fault was "probably" something!

ILS FROB Concludes GLONASS Proton Launch Anomaly Investigation
The ILS Failure Review Oversight Board (FRO:cool: concluded its review on August 13, concurring with the conclusions of the Russian State Inter-agency Commission on the root cause and the associated corrective action plan of the July 2, 2013 Proton M/Block DM mission failure with three GLONASS navigational satellites for the Russian Federal Government.
The members of the FROB agreed with the findings of the Russian investigation that the root cause of the failure was due to the improper installation of the three yaw angular rate sensors located on the Proton launch vehicle, which caused the vehicle to deviate from its flight path shortly after lift-off.
“We very much appreciate the time, effort and participation of our customers, the insurance underwriters and technical experts in the FROB process. They worked tirelessly with us to ensure that the review was conducted thoroughly. As we work towards the return to flight of the Proton vehicle, we thank all of our customers for their continued support,” said ILS Vice President of Programs and Operations, John Palmé.
The ILS Proton return to flight mission will be the Astra 2E satellite for SES on September 15, 2013. The scheduling of the remainder of the ILS Proton near term manifest for 2013 is currently being determined

Exactly a Month tomorrow then. Looks like we could see 2E take over live services by mid-November following the usual in-orbit testing, guess that will please SES who at least now have a revised date for launch.

Roscosmos denied information about the launch of "Proton" September 15

Baikonur. August 15. INTERFAX.RU - Head of the Russian Federal Space Agency Anatoly Baikonur Belokon denied the alleged scheduled for September 15, starting from the Baikonur rocket "Proton".

"Until we solve the question of liquidation of consequences of failure, we can not talk about new launches," - said Belokon.

So he responded to a request to comment on the recent media launch date of "Proton", expressed by the Minister of Environmental Protection of Kazakhstan Nurlan Kapparov at a meeting of the government commission on the elimination of the consequences of falling rocket "Proton-M".

Information about what the next heavy launch rocket "Proton" is scheduled for September 15, published on the website of ILS on Wednesday.
